Use Case Submission Form

This document is intended to help you document a basic use case, or user story, related to health information exchange. Below is a guide and questions to help you capture this information as a user. You can also use this form to articulate your own needs. Feel free to direct your answers however you see fit, just remember, the goal is to understand why users need health data, and thus, the exchange of health information.

The purpose of this use case is to understand your role and how you use health data to support your work. It will ask you a series of questions aimed at understanding how you rely on health data. This information will be used by the groups that work to facilitate health information exchange (i.e., making health data available at the right time in the right way) as they must understand users' needs to make smart decisions about how data is made available.

In this form, please think about what you do today as well as the ideal future state.

What is health data? Health data is any information that represents the health and well-being of our residents. For reference, think about the data that’s logged in electronic health records when patients receive care or the information a case worker may capture when trying to coordinate care and ensure a patient receives both the medical and social supports he/she needs.
