Episodic Care Management Use Cases

Patients require episodic care management when they utilize a provider or health care organization that they do not have a pre-established health care relationship with, such as care received during visits to anemergency room or walk-in clinic. Episodic care providers can struggle to gain access to patient data since it is often not available within the database for their organization. Gaining access to patient data via a Health Inforamtion Exchange allows providers to make more informed decisions about a patient they may not be familiar with.

Persona Story

 Doug is the CIO of a treatment center, who would like data access via hospital or elctronic health record through a Health  Information Exchange when initiating new patient intake, managing chronic coniditions, and ongoing patient care. Sue has struggled to bridge the gaps between Vermont & New Hampshire, due to limited access of data for Vermont residents seeking care in New Hampshire. Currently only one facility provides the real time data that Sue is seeking. The lack of available data between states hinders the best possible care. Access to up to date patient data enables providers within her multi-state network to make more holistic decisions for patients seeking care in more than one state.

Longitudinal Care Management Use Cases

A Longitudinal Care Plan takes into account the holistic needs of a patient or client, aggregating information from various providers, care visits, and incorporating the patient's unique health care goals over time. Longitudinal Care Management depends on networks of data sharing to generate the "big picture" of a patient, taking into account behavioral, mental, and physical needs and goals.
