I am presenting you with an opportunity to share your thoughts with the State of Vermont, Department of Vermont Health Access. Your responses will be considered and incorporated into the Vermont State Medicaid Health Plan (SMHP). This document informs the future of Vermont’s health data interconnectivity as we strive to attain the best possible clinical outcomes, efficiency, privacy, and security.
You may "choose your own adventure" while completing the survey by only answering questions relevant to your organization.
I have included questions aimed towards a variety of provider groups, such as:
- Offices that exclusively use paper health records.
- Offices that have already implemented electronic health records (EHRs).
For all groups, there are questions centered around the state's Health Information Exchange.
Upon completion, you may learn that it is possible for your organization to qualify for incentive payments to implement an EHR system. You may also get a glimpse into the future of patient records, clinical data, and Vermont’s health information exchange.
