The EHR Incentive Program Awareness Survey

This survey is being conducted by the State of Vermont Department of Vermont Health Access in order to gauge awareness of the Vermont Medicaid Electronic Health Record Incentive Program (EHRIP) and Meaningful Use requirements. As a practitioner (or a practice representative) who provides services to Medicaid beneficiaries, you are potentially eligible to participate in the Vermont Medicaid EHRIP. 

Eligible Professionals can receive up to $63,750 for full participation in the program, over six years. This includes a payment in the first participation year of $21,250 to adopt, implement, or upgrade (AIU) to a Certified Electronic Health Record Technology (CEHRT) system.

The deadline to qualify for participation in the Medicaid EHR Incentive Program is December 31, 2016. Providers will have until March 31, 2017 to register with CMS and to submit their applications.

Your responses to the short survey below will help you determine your eligibility, and provide the Vermont EHRIP Team information about readiness or barriers for participation.

Note: Individual providers are eligible for payment, not practices as a whole. As you step through the survey, answer on behalf of yourself or any other provider who may meet the criteria.

1. Are you aware of the Medicare or Medicaid Electronic Health Record Incentive Programs?
2. Individual practitioners eligible for incentive payments must perform less than 90% of their services in an inpatient hospital or emergency room setting. Do you, or at least one of the providers you represent, meet this criteria?
3. Are at least 30% (20% for pediatricians) of your patient encounters furnished to Medicaid beneficiaries?
4. Are you, or at least one of the providers you represent, a Physician (MD or DO), Dentist, Certified Nurse-Midwife, or Nurse Practitioner?
5. Does your practice use an Electronic Health Record System?
6. If "Yes," is it a Certified EHR System, as defined by CMS and the Office of the National Coordinator for Health Information Technology (ONC)?
7. If "No," do you plan to acquire Certified Electronic Health Record Technology (CEHRT) by the end of Calendar Year 2016?
If you answered "Yes" to questions 2 - 4, and utilize a CEHRT, or plan to acquire a CEHRT before 12/31/16, you may qualify to receive incentive payments. Please see our webpage to learn more about registering for the Vermont Medicaid EHR Incentive Program.
