Connected Health Symposium 2016
At the Connected Health Symposium, look forward to discussions that emphasize real-world examples of how connected health is having an impact on improved patient engagement, care outcomes and highlighting successful business models. While the conversion to electronic health records and health information exchange will deliver greater efficiencies and savings, the transition has been complex and challenging for patients, providers, and health care organizations alike. Networks like the Vermont Health Information Exchange (VHIE) provide a platform to support change, while ensuring that electronic health information is secure and readily available when Vermonters need it.

Objectives
- To provide a forum for national subject matter experts (SMEs), state program leads, and other key stakeholders to share updates and learn from a regionally focused gathering.
- To encourage regional networking, collaboration, and information sharing around the Medicaid EHR Incentive Program and other health IT priorities.
